Job Summary
As a Home Health Physical Therapist, you will deliver independent, age-appropriate physical therapy services to patients in their place of residence. You will develop and coordinate individualized care plans, provide education to patients and families, and ensure high standards of treatment and documentation in accordance with agency policies and state/federal regulations.
Key Responsibilities
- Conduct comprehensive patient evaluations to determine the extent of dysfunction.
- Identify early warning signs of complications, including skin integrity and cognitive changes.
- Differentiate between emergent and non-emergent patient conditions and escalate when necessary.
- Establish treatment goals and design plans to achieve them.
- Provide direct therapy, patient/family education, and ongoing reassessment.
- Initiate discharge planning when goals are met.
- Accurately document patient progress and maintain compliance with OASIS guidelines.
- Coordinate care with pharmacy, inpatient/outpatient departments, and other healthcare professionals.
- Supervise Physical Therapy Assistants and Home Health Aides as per regulations.
- Support staff education, departmental programs, and quality improvement initiatives.
Basic Qualifications
- Minimum of two years of physical therapy experience.
- Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or Doctorate).
- Active California Physical Therapist License.
- California Driver’s License and clear driving record.
- Basic Life Support (BLS) certification.
- National Provider Identifier (NPI).
- Strong ability to assess patient conditions holistically.
- Effective written and verbal communication skills.
- Must be able to work in a Labor/Management Partnership environment.
Preferred Qualifications
- Experience with automated clinical information systems.
- One year of home health agency experience.
- OASIS certification preferred.
